Appointments:
Appointments are made from physician referrals. Your primary care physician will notify us of your need for an evaluation. Our office will notify you of your appointment and send you an information packet. There are several forms that you must complete and return before your appointment.
Health Insurance:
We participate in most health plans, HMOs and PPOs. Please verify that our physicians are in your health plan network before making an appointment. Some health plans require that you have authorization (referral from your primary care physician) before seeing a neurosurgeon. It is your responsibility to see that the referral is complete and in our office before your scheduled appointment. Failure to have this done might result in your appointment being rescheduled.
